The most important requirements are: (1) Opioid addiction for at least two years (2) Previous substitution therapy unsatisfactory (Other drug use, injection use) (3) At least 6 months in substitution therapy (4) At least 23 years old, or from 18 years under special conditions (5) Counseling is mandatory at the start of treatment
